Sunrise Terrace Hosts Annual Mini-Convention for Staff

The Iowa Health Care Association (IHCA) holds an annual convention each September that Sunrise Terrace management staff attend to keep up with the latest changes in long term care. We often come away from the three day education with our arms full of new information and ideas to implement in order to provide the best care to our residents and go above and beyond to meet their needs. It is great that management staff come away with more knowledge and have ideas to make changes within their departments, but this knowledge must reach the front line workers in order to make a difference for our residents. Therefore, we hold our own mini-convention for all staff and bring the same fun-filled, positive experience that IHCA provides its attendees.

This year’s theme was Face the Challenge, Create the Magic – a very fitting theme for all the health care changes coming our way. In spite of the challenges we are currently facing, it is our job to ensure we create magic for our residents each and every day.

We held two education sessions, one in the morning and one if the afternoon. Each of which consisted of three, fifty minute classes. Our first class was on the importance of overall quality and the direction we must take in order to stay ahead of our competition and provide the best possible care for our residents. The second class was on Quality Assurance Performance Improvement (QAPI), a new regulatory process we must follow to address quality issues and concerns. The last class was on antipsychotics reduction and the interdisciplinary team approach that must be taken to keep our facility’s antipsychotic rate low and keep our residents quality of life at the highest level.

A delicious breakfast was served by our talented Dietary Manager, Abby Tuttle, with homemade cinnamon rolls, chocolate muffins, and fresh fruit. Lunch was equally as delicious as we provided Subway sandwiches, three choices of coup, cookies, and homemade chex mix that prepared by our very own Activities Coordinator, Rhonda Connop.

And then, the fun began. Those who followed directions and successfully registered for the morning and afternoon sessions were dealt a card from a fresh desk of cards, where half of it was cut and placed into a basket. Three lucky winners were then drawn from the basket and came on stage to answer four questions in regards to our handbook. For each question they answered correctly, they received a rabbit to throw into a giant magician hat. Then, for each rabbit they made in the hat they won a prize.

It was a fun day and we plan to hold a sixth, award winning, mini-convention for all staff in 2016!

Our three winners were Christina Huss (Dietary Services), Jenny Jacobs (Housekeeping), and Sam Trejo (Nursing). The room was crowded, and as you can see, one of the best views was through the chapel windows as our vendors and employees watched with excitement.

Get Well…Go Home Success

Get Well…Go Home, Sunrise Terrace Nursing & Rehabilitation Center’s skilled rehabilitation program, has successfully returned 114 patient’s back to their home. From inception in 2011, this translates into an 83.3% success rate – a statistic that our team is very proud of!

For those of you who are not familiar with this program, it is a Medicare Part A benefit that is acquired after a three midnight hospital stay. If you or a loved one go home after a qualifying stay in the hospital and are unable to get back on your feet like you’d like, you are still able to rehabilitate with us and our dedicated therapy team if it is within 30 days of your discharge!

Not from the area, you say? Mark our word, our rehab difference is worth the distance! For all diagnoses over the last quarter, our average length of stay for skilled rehabilitation was 27 days – 16.8 days less than our competitions! Our team is committed to getting you well and back to your home, where you belong. Choose us, where we “Get Well…Go Home” sooner!

Porter Rallies & Recognizes Certified Nursing Assistance at Sunrise Terrace

Sunrise Terrace Nursing & Rehabilitation Center requested the honor of Co-Founders, Lori Porter and Lisa Cantrell, of the National Association of Health Care Assistants, NAHCA, to recognize their community’s caregivers and care team for their dedication to providing high quality, compassionate care for the frail and elderly in their facility.

Following an eventful, “Rocky” music themed, outside rally Lori spoke on several topics; one of which was how to embrace nursing homes, not tear them down. “The only time you hear about nursing homes in the media is when abuse and neglect occur”, stated Porter. “However, I think this is one of the best all-around places to come to – if you want to see quality in action, this is the place.”

Nursing home employees are often criticized for where they work, intentionally, or not. They are often told, “I don’t know how you can do that” or “It takes a special person.” You bet it takes a special person to work in a nursing home – it is the most regulated business in the entire world, second to nuclear power. All employees within the nursing home are held to the highest standards of any business or any other health care related organization. Now that is something to be proud of – they do a job other people won’t do! Nursing home employees are proud to serve their elders, and are the first to assist other elders in the community when they need assistance.

The image is that it is not “cool” to work in a nursing home and this must change. It is the goal of NAHCA’s multi-state tour, “CNAs Caring for an Aging America, Proudly Taking It On” to make a difference and change perceptions. According to Lori, who has been in many facilities across the nation, “If there is anyone who has the ability to be one of the greatest teams out of the 15,000 nursing homes in the country, it is Sunrise Terrace!” “Furthermore, it is rare to find a facility where the administrator has been around as long as Peggy, who still cares as much as she does.”

Porter ended her message with the statistic that Iowa is the 4th fastest growing state in the nation for elders in three categories – 65+, 75+, and 85+. Not only Iowa, but the rest of the nation will need to be prepared to care for these elders. Each facility will need a team approach and culture to address resident needs and it is NAHCA’s goal to make all nursing homes great so that all elders have an exceptional place to live and be cared for.

LaVonne Smith, a former CNA and current Sunrise Terrace resident, sat down for an interview with Lori and Lisa. When asked how she become a CNA, she replied, “I needed to do something that I enjoyed. I took a job living in a home taking care of two ladies. I really enjoyed it, talking to them and taking care of them. It just made me feel special. So, then I decided to take a CNA class to get my certification. My care was from my heart!”

Medicaid Recipients and Family Members

On Monday, December 7th at 3:00 and 6:00 PM we will have an informational meeting on the new Medicaid Managed Care process that will go into effect on January 1st, 2016. Monthly Support Group

On Wednesday, December 16th we will have our monthly support group for family members and friends of those with dementia. Our confidential support group is available to discuss coping strategies, provide information and resources, and allow participants to share their experiences with one another, shall they choose to do so. Medicaid Modernization

You may have begun hearing this term in the news recently. What is Medicaid Modernization? It is an initiative announced by Governor Branstad on February 16, 2015 that aims to improve access and care coordination, promote accountability and outcomes, and provide a predictable and sustainable Medicaid budget for the state of Iowa. It is called IA Health Link. Through Medicaid Modernization, the Iowa Department of Human Services will contract with managed care organizations for the delivery of health care services for most Medicaid members. A managed care organization (MCO) is a health plan or type of health coverage that will make sure you get complete care that’s just right for you from the managed care organization’s network of providers.

Sunrise Terrace Nursing and Rehabilitation Center and all other providers will be required to contract with MCOs. We have begun this process. Current Medicaid members do not need to do anything at this time. You will have the opportunity to choose your MCO in the coming months. Plan to get a notice soon to let you know when you will need to make a health plan choice.

Coverage by the MCOs will begin on January 1, 2016. For more information about this IA Health Link transition program visit dhs.iowa.gov.

 

 

Bike Rodeo

Sunrise sponsored a Bike Rodeo for children in the community to help raise awareness of bicycle safety. Dave Norcross of the Winfield Police Department gave a program on rules of the road, hand signals, and ended the program with an obstacle course. Prizes included brand new boy’s and girl’s bikes and Kernels baseball tickets! Winners were Andrew Brown and Beatrice Anderson.

 

Car Show

Gary Long and Gary Huston gave us a preview of the Crooked Creek Day car show when they brought out an antique truck and two cars for the residents to check out. These beauties brought back many memories and spurred lots of reminiscing.

 

Get Well…Go Home Statistics

Sunrise Terrace Nursing & Rehabilitation Center has had tremendous success with their Get Well…Go Home program since the program began in 2011 and we would like to share some of our recent statistics with you!

Over the past 60 days, our average skilled stay was 10.67 days!

Our readmission rate to the hospital for the past 90 days is 0%. This means, during a patient’s skilled, short-term stay, we were able to keep them well within our facility without receiving additional assistance.

To date, we have successfully returned 112 patients back to their home!
